Steps in compiling your portfolio include:

1. Verify that you are using the appropriate portfolio for the Authorization for which you are applying.
   
   
2. Complete the Application for Authorization.
   This form provides background information about you and will eventually be submitted along with your portfolio to the State Review Team. This form must be completed by all applicants and be signed by your supervisor.
   
   
3. Complete your portfolio. Steps in completing your portfolio include:
   • Using the Mastery Profile Key, identify the level of mastery required for each specific competency.
     
     

     As shown in this example, the mastery level for this competency is 4, or demonstration of the competency at the application level. As such, you must provide documentation that you "can demonstrate this skill with guidance and are able to use the skill or knowledge in a variety of situations".
     
     

   • Using the Document Key identify the appropriate type of documentation to support the mastery requirement for each competency.

     The Documentation requirement for this competency is indicated by the underlined numbers,
     4 - (a written work sample produced on the job such as an IFSP) or
     7 - (a written document describing experience and specific examples related to the competency).

     These numbers represent the acceptable Documentation that must be provided for this competency. Remember you only need to provide documentation for one of the underlined requirements.
     
     

   • After determining the type of documentation required for each competency, circle the type of documentation being submitted.

     As you will note in the example above, the applicant has circled the number 4 in the Documentation section. This indicates that the applicant has chosen to submit a written work sample for documentation of the competency. You should circle the documentation that you are providing for each competency.
     
     

   • Compile your documentation and place into the appropriate competency section of your portfolio.
